The original Birth Certificate of the applicant, legalized by the Peruvian Consulate or Consular Representative of Spain accredited in Peru and by the Ministry of Foreign Affairs. |

To be update in the payment of the Annual Rate of Immigration and the valid residence. |

An Affidavit in which the applicant states to have a good health, address and, also, criminal, judicial and police record. |

The applicant must be over 18 years old and has two consecutives (02) years as a minimum of legal residence in the country. |

Original and copy of the passport. |

dos (2) clear colored passport size photo with white background. |
|
Attach the documents according the valid immigration status. |

Receipt of payment at Banco de la Nación by the issuance of Titles, equivalent to 5% - UIT.(S/180.00). |

To return the valid Alien Registration Card at the moment of the delivery of the Title. |
