General Requirements

Form F-006 (free of charge).

Receipt of payment - Banco de la Nación for procedure (S/.46.55).

Receipt of payment - Banco de la Nación for registration, equivalent to 1% - UIT.(S/36.00).

Application addressed to DIGEMIN according the established format.
  In case of being spouse indicates the maiden forenames and last name.

Certificate of Marriage registered at Public Registry - RENIEC, in which indicates two (2) years of marriage.
  If the marriage was celebrated abroad, the applicant must present the certificate and a document that proves the registration in the Consular Office, legalized by the Ministry of Foreign Affairs.
  If the marriage was celebrated abroad and it was not registered in the consular office, the said event would be registered within the period of ninety (90) days of the applicant's arrive to the country in the registry of the municipality that the applicant states as a domicile, after the established term, the applicant have to do it by judicial means.

A copy of the Alien Registration Card indicating the two (2) years of residence.

Birth Certificate and the respective copy of the spouse.
  In case that the spouse has the Peruvian Nationality by Naturalization or by option, the applicant will submit a copy of the Title of Register.

A copy of the Identity Document of the Peruvian Spouse.

An Affidavit in which the applicant states to have a good health, address and, also, criminal, judicial and police record.

One (1) clear colored passport size photo with white background.

Important Information
Payment for Procedure: S/. 46.55 nuevos soles.
Qualification: Required Evaluation.
Procedure Timeline: 15 days.
Department in Which Starts the Procedure: DIGEMIN or Immigration Headquartes.
Authority Who Approves the Procedure: Director General de la DIGEMIN y Director de Naturalización.
Authority in Case of Reconcideration:Director General de la DIGEMIN y Director de Naturalización.
Authority In Case Of Appeal: Ministro del Interior.
