Change of Consulate to the Reception of the Visa

General Requirements

Form F-007 (free of charge).

Receipt of payment - Banco de la Nación for the procedure (S/. 26.95).

Submit an application in which state the reason of the change and the address of the new consulate.

A copy of the identity document of the applicant.

Note:

It will be granted just for once, within the following three month communicated the approval of visa to the Ministry of Foreign Affairs.

Payment for Procedure: S/. 26.95 nuevos soles.
